Referral Requests

• Please send medical records, imaging, and patient history to our email at [email protected] 

• Please call our clinic for the more complex cases to ensure that we are equipped for their care.

• After the patient's discharge, we will send medical records to your clinic once they are completed. 

Transfer Requests

• Please fill out the Transfer Sheet with a treatment plan, including times meds were given and dosing (in mg). Click HERE to print the transfer sheet.

Diagnostics We do not Provide

• All City Pet Care South VEH does not have the capabilities to take Magnetic Resonance Imaging (MRI), Computed Tomography (CT), or Echocardiograms.

• The emergency hospital only has X-ray and Focused Assessment with Sonography for Trauma (FAST Scan) capabilities.

• Full ultrasounds will need to be scheduled with our general practice.
